Who We Are And What We Do
The mission of the California Hospice Foundation (CHF) is to support quality hospice and palliative care through providing education to individuals and the communities who care for them.
We envision a future where every person understands the value hospice brings to care at the end-of-life, and is able to experience hospice, if they so choose.
When asked, in public opinion polls, the overwhelming majority of Americans say they want hospice at end-of-life. However, only about 37% of us actually are referred to and receive hospice. We know that many individuals with end-stage diseases are never even offered hospice. CHF believes that when the time comes, each of us deserves to have thought about end-of-life decisions and to have a choice about how to spend our last days.
To this end, we strive to improve access to quality hospice and palliative care services through research, education and public awareness.
Our services include:
- Programs for professionals that advance high quality end-of-life care;
- Telephone information/referral and support for individuals and their families at one of the most difficult times in life;
- Educational programs and printed materials for the general public that enhance awareness and understanding of hospice and palliative care;
- Research that clarifies systems and demonstrates the effectiveness of hospice and palliative care to reduce pain, manage symptoms and illicit positive patient and family satisfaction;
- Collaboration with community groups and health care professionals, knowing that through working together we can truly effect change.
